Edema and Chronic Kidney Disease

Edema refers to retention of excess fluid in tissue space or body cavity, it contains hydrops and dropsy, and manifested as generalized edema and local edema, how edema occurs is related to many factors, usually it is lesions on some important systems organs, kidney disease is one case that is not to be ignored, it is called renal edema, which is the early sign of chronic kidney diseases.
 
How the edema related to CKD is mainly attributed to two causes. First, there are swelling and hyperplasia occur in the glomerular capillary endothelial cell and mesenchymal cell, which caused the glomerular decreased in blood volume, meanwhile the kidney tubules are reabsorbing water normally, consequently there will be little or no concentrated urine discharged, the fluid will be retained in blood and tissue space, and patients will commonly experience generalized edema, the CKD routinely had reached its mid-to-late stage under this situation.
Second, the permeability of glomerular capillary wall can increase because of inflammation, which lead to plenty of albumins getting through the basement membrane, then the plasma osmotic pressure decreases and water will move into tissue space and causes edema, in this case, the edema usually occurs in soft parts of the body, like eyelids, face, or ankles, and it can be more obvious in the morning, this condition is likely to occur in the early stage of CKD.
There also some other factor that contribute to the occurrence of edema in CKD patients, to determine whether the edema is related to CKD, people should have a test on urine, if there is proteinuria and occult blood urine occur, that is possible related to kidney disease.
