Questions

I would like some help on something I could do to help my feet. The doctor said it was restless leg syndrome.
They burn up and I cannot keep them still – I cannot sleep or watch. I was prescribed clonazepam but nothing happened at all. I even took four one night, I was so desperate, but they don't work. I don't bother taking them anymore.
I also have kidney failure due to renal tuberculosis. My phosphate level was 3.5 but now it is back down to 1.6, so it cannot be that, my creatine at present is 698.
I will not start dialysis for a few weeks. Do you think it is possible the leg problems will calm down with dialysis? And is there anything you can maybe suggest to do for a little relief? Is there anything I could bathe them in?
Or anything you could suggest I ask my doctor for? I would really appreciate your opinion.

Answer

Thank you so much for your email, and I do sympathize with your problem.
Restless leg syndrome affects up to 15 per cent of the general population and it leads to this terrible burning, pins and needles, and restless feelings in the muscles of the leg, often at night, but potentially present at any time of the day.
You obviously have enough to contend with already with your renal failure, and I think you are absolutely right in suggesting that your underlying problems have something to do with the restless leg syndrome in your case and that it could well calm down once your dialysis has started.
My research on your formidable list of medications tells me that tacrolimus can cause pins & needles and sensory disturbances.
Fluvastatin is contraindicated , in other words should not be taken, when there is severe renal impairment and when there is unexplained aching in the muscles, muscle tenderness or weakness – this should be reported to your specialist immediately.
Doxazosin can also cause fluid retention, which is a possible pre-disposing cause of RLS, and amlodipine can also cause muscle cramps.
This however is a calcium antagonist type of drug that strangely enough is sometimes used to treat RLS, so this is unlikely to be the cause.
Prednisolone, being a steroid, can cause an imbalance in the minerals in your blood with loss of potassium, and it can also cause muscle weakness and fluid retention in its own right.
All of the above-named drugs are possible reasons why you might be having trouble, but the most likely cause however is rifater, also know as Isoniazide, which some people break down more slowly in their bodies than others.
In large doses this frequently causes the side effects of insomnia, restlessness and muscle twitching. For this reason it is often given with pyridoxine, which you are already on, to minimise these problems.
The dose of pyridoxine could possibly be increased, and I would very much encourage you to talk to your specialist about doing this. Many of your drugs are necessary and should not be adjusted without consultation.
You might also like to consider having frequent massage for your restless legs, raising the foot of your bed to inhibit fluid retention, and the wearing of elasticated stockings for compression purposes.
And in the suggestions of the therapy is that you can choose Chinese herbs to adjust your kidneys. Micro-Chinese Medicine Osmotherapy is a good choice for your kidney disease .This therapy can make the Chinese herbs infiltrate your kidneys directly. So this therapy can take effect quickly than other Traditional Chinese Medicine .

This is probably a longer opinion than you asked for, but then I am very opinionated! I hope it helps.
