Some renal patients wake up in the middle of the night because of an uncomfortable sensation they feel in their legs. Their legs feel itchy or tingly, or may jerk or move on their own, waking them up. People may even experience leg cramps while they are awake, usually during a hemodialysis session. This is called restless leg syndrome. It often occurs when the legs are at rest, such as when a person is sitting or lying in bed.
Mild cases of restless leg syndrome occur periodically, while moderate to severe cases can occur regularly. The sensation varies from patient to patient. It can be irritating, itchy or painful. Some patients find that moving their legs makes the uncomfortable sensations lessen or go away entirely. Because restless leg syndrome occurs when you are trying to relax or fall asleep, it can leave you feeling tired from lack of rest.
Doctors have pinpointed several factors that can contribute to restless leg syndrome. They are:
Iron deficiency or anemia
Caffeine, alcohol, tobacco
Diabetes
Certain anti-nausea, anti-seizure and allergy medications

These non-breathing intervals are called apneas. They are usually followed by sudden attempts by your body to breathe. One of the symptoms of apnea is snoring. Patients with sleep apnea often snore heavily. The snores continue until breathing is interrupted or stops, which signals an apnea. Hemodialysis patients should have a target Kt/V of at least 1.2. Peritoneal dialysis patients should have a Kt/V of at least 2.1. In order to meet the target Kt/V, you should not skip dialysis sessions. Missed dialysis sessions can mean a build up of waste and fluid, leading to other complications besides sleepless nights.

Emotions

Worry, anxiety and sadness can keep you up at night. Also, if you find yourself able to fall asleep, but then awaken in the early morning hours unable to return to sleep, this is a sign of clinical depression. Talk with your doctors and social worker. They are there to help. If your sadness, anxiety or depression lasts more than two weeks, tell your doctor immediately.
Changes in your sleep pattern
Sometimes patients who have chronic kidney disease are more tired than usual. They tend to fall asleep earlier than their normal bedtime or nap during the day. This could affect your ability to fall asleep or stay asleep at night.
Although naps can be refreshing, naps that are too long can interfere with your ability to fall asleep and stay asleep at night. If you are tired and feel that a nap can help you, limit your nap time. Naps that last longer than an hour can disrupt your sleep cycle.

Caffeine

Anemia in Patients With Chronic Kidney Disease

Anemia, as defined by the NKF, is a hemoglobin (Hb) concentration < 12 g/dl for women and < 13.5 g/dl for men.3 Conversely, the European Best Practices Guidelines for the Management of Anemia in Patients with Chronic Renal Failure defines anemia according to age and sex. Anemia is defined as an Hb concentration of < 11.5 g/dl in women, < 13.5 g/dl in men ≤ 70 years of age, and < 12 g/dl in men > 70 years of age.4 In patients receiving dialysis, the recommended Hb target value is ≥ 11 g/dl in women and ≥ 12 g/dl for men. Regardless of the definition, anemia is a common complication associated with CKD.

Causes of Anemia

Diabetes is one of the most common causes of CKD. Although patients with diabetes are regularly monitored for a variety of complications, such as neuropathy, nephropathy, and retinopathy, Hb concentrations frequently are not routinely assessed. Interestingly, reductions in Hb often occur before the onset of overt diabetic nephropathy.
This reduction in Hb occurs for a variety of reasons. Approximately 90% of the hormone erythropoietin is produced by the kidneys. Under normal physiological conditions, hypoxia in the kidney leads to an increase in the production of erythropoietin, which subsequently stimulates erythropoiesis.8 The kidney, in turn, senses increased oxygenation because of the formation of the new erythrocytes and decreases erythropoietin production. However, tubulointerstitial damage associated with diabetes occurs early in the course of diabetes, even before a reduction in GFR or albuminuria is noted.9As functional renal tissue declines in patients with CKD, the body is unable to produce adequate amounts of erythropoietin in response to hypoxia in the kidney.8 Another factor commonly seen in patients with diabetes is the use of medications that may adversely affect Hb production. These include metformin, fibrates, thiazolidinediones, and angiotensin-converting enzyme inhibitors. Finally, systemic inflammation associated with microvascular disease in patients with diabetes leads to the production of inflammatory mediators, such as interleukins and tissue necrosis factor. These mediators blunt the effect of erythropoietin on the bone marrow, where erythroid precursors are stimulated.9
Other factors, although not specific to patients with diabetes, further exacerbate anemia in patients with CKD. These include platelet dysfunction leading to an increased risk of gastrointestinal bleeding, shortened erythrocyte survival time (30–60% of the normal 120 days), and hemolysis secondary to uremic toxin accumulation. In patients receiving dialysis and especially those on hemodialysis, chronic blood loss resulting from frequent phlebotomy for laboratory studies and loss of blood in the dialysis tubing and dialyzer after each hemodialysis treatment may also contribute to declining Hb values. Finally, malnutrition and deficiencies of iron, folate, and vitamin B12have been found to cause a reduction in Hb concentrations.10

Impact of Anemia

The impact of anemia on patients with CKD is profound. In addition to the well known symptoms of fatigue, dizziness, and shortness of breath, anemia has been associated with more severe adverse outcomes, such as cardiovascular complications including left ventricular hypertrophy and congestive heart failure. In patients with diabetes, anemia has been associated with a decline in kidney function, which often occurs in patients with diabetes. Hypoxia caused by anemia stimulates the renin-angiotensin-aldosterone system and contributes to renal vasoconstriction. These factors further exacerbate proteinuria by increasing protein in the renal tubules in patients with diabetes.9 Of note, in patients with type 2 diabetes, anemia has been shown to be an independent risk factor associated with the loss of kidney function.11 In patients with diabetes, anemia can also contribute to the severity of cardiovascular disease and independently increase the risk of retinopathy. Anemia is also thought to hasten the progression of diabetic neuropathy.11 Other general complications associated with anemia include reduced cognitive function and mental acuity, impaired quality of life, and the need for blood transfusions.12–15
Correction of anemia has been shown to improve cardiac function possibly by reducing exercise-induced myocardial ischemia.12Treatment of anemia associated with CKD has also been shown to result in improvements in exercise capacity; physical performance features such as endurance; energy; and physical mobility.13 Patient satisfaction increases when anemia is corrected, as evidenced by higher quality-of-life scores, improved sexual function, better cognition, less depression, and better socialization.14 In non–dialysis-dependent CKD patients, stabilization of renal function has been associated with treatment of the anemia of CKD.12 Finally, treatment of anemia has been shown to reduce hospitalization and mortality rates.

As a result of the potentially severe consequences of anemia in CKD, early recognition and management of anemia are imperative. Consequently, monitoring Hb and detecting anemia in patients with diabetes is essential.

Protein Intake and CKD

Diet is one of important factors in your kidney disease health.I believe that every kidney disease patients knew the importance of high quality protein for their kidney problem .So I want to introduce the relationship between Protein intake and CKD.

The amount of protein you can have depends upon the progression of the disease. If you're in the earlier parts of kidney failure, you'll probably have to limit the amount of protein you can consume. That's because your kidneys will have trouble eliminating urea, which is a waste product of protein metabolism.
Protein is essential for repairing and maintaining muscles, organs and other tissues. You also need some protein to keep your immune system functioning properly. Reducing your protein intake will take some of the load off your kidneys, but, you'll still need some protein, probably about 1 gram protein per kilogram of bodyweight, or less, per day. Ask your health care provider exactly how much protein you can consume. You may also need to speak with a dietitian or nutritionist who specializes in kidney disorders. If your disease progresses to the stage where you need dialysis, you'll probably need to increase your protein intake again, up to 10 ounces of high-protein foods per day. But you need to be careful about which types of proteins you choose. Some of the plant-based protein sources are also high in potassium and phosphorus, two minerals that you may still need to limit. These plant-based protein sources are nuts, seeds, peanuts, legumes and lentils. Grains and animal based protein sources should be fine.

High Protein Foods

In order to monitor your protein intake, you'll need to know which foods are highest in proteins. In general foods from animal sources contain the most protein. So poultry, meat, seafood, dairy products and eggs are generally high in protein.

CKD in older people

The majority of patients diagnosed with Chronic Kidney Disease (CKD) are elderly and CKD is linked with poor cardiovascular, cognitive, and disability outcomes in these people. Only a minority of these patients will progress to end stage renal disease (ESRD) while the majority will die due to cardiovascular disease. Thus, only a small number of these patients with CKD will benefit from specialist nephrologist assessment. The priority for the remainder should be cardiovascular disease prevention. We have reviewed specific issues relevant to older people to determine high-risk groups with CKD that are likely to benefit from a more intensive risk reduction intervention and to allow identification of clinically relevant renal disease.

Introduction


The prevalence of Chronic Kidney Disease (CKD) has been rising and is causing a global challenge as a non-communicable epidemic. In the UK, the prevalence is about 10% of the po­pulation, and in people aged over 65 years, this figure reaches 40%. Risk factors predis­posing the elderly to CKD remain unknown. However, most analyses show that the majo­rity of patients affected either with microalbu­minuria (MA) or with reduced glomerular fil­tration rate (GFR) are the elderly (> 65 years). Cardiovascular disease (CVD) has a significant impact on CKD in the elderly and affects CKD progression in this age-group. It is well known that renal function declines with age. It is still not clear whether CKD in older people is merely due to age-related changes in kidney function or a genuine kidney disease that will progress to end-stage renal disease (ESRD). Nevertheless, we already know that a minority of elderly patients with CKD will progress to ESRD and that the majority will have mor­bidity and mortality related to CVD, rather than ESRD. However, labeling older, frail pa­tients with a significant disease can be wo­rrying for them, especially if that will include asking them to travel long distances for spe­cialist secondary and tertiary services. With these above concerns, there has been evolving national and international guidelines to guide primary care physicians and non-renal specialists to refer patients according to the CKD stage. Most of these guidelines have overlooked the age stratification and they have been cri­ticized for building guidance based upon esti­mated glomerular filtration rate eGFR alone (calculated by The Modification of Diet in Renal Disease [MDRD] equation, which has not been validated in patients above the age of 70 years). Currently, there are no other more accurate methods to estimate GFR in older people with CKD. Another marker which has been found to be superior to serum creatinine in measuring GFR is cystatin C, which may have future implications for older people.
The majority of elderly patients with CKD are in stage three that could well be managed in the community by primary care physicians. The most important issue is to address the high CVD risk in this population which is confoun­ded by the co-existence of CKD. In addition, CVD is known to accelerate CKD progression in elderly subjects. The importance of early detection and prevention has been the focus of many national and international guidelines and standards frameworks. Finding a simple and feasible screening tool for early CKD detec­tion has been an attractive goal.
Microalbuminuria and low GFR are very co­mmon in individuals > 65 years, which contri­butes to the large prevalence of CKD reported in this age-group. There is evidence that MA is a strong predictor of CVD risk in the general population. It is not clear, however, whether MA in early CKD is a marker of kidney da­mage or widespread CVD including atheroscle­rosis and peripheral vascular disease, which are not uncommon in the elderly.

We performed a search of Medline and Embase from January 1969 to September 2009 using keywords relating to CKD in older people and vascular and renal outcomes. Only English language articles were selected. Articles were reviewed for relevance by abstract. A manual review of citations in retrieved articles was performed in addition to the electronic lite­rature search. The final list of cited references was chosen on the basis of scientific quality and relevance to the topic of review.

Chronic Kidney Disease is defined as either a low GFR rate of less than 60 mL/min/1.73 m 2 , or normal GFR plus evidence of kidney da­mage (most commonly albuminuria, hematuria or abnormal renal ultrasound for three or more consecutive months) regardless of age.The severity of CKD is classified according to the level of GFR, regardless of the etiology, into five stages:
Stage one: kidney damage with a normal or increased GFR (> 90 mL/min/1.73 m 2 );
Stage two: kidney damage with a mild decrease in GFR (60-89 mL/min/1.73 m 2 );
Stage three: a moderate decrease in GFR (30-59 mL/min/1.73 m 2 );
Stage four: a severe decrease in GFR (15­ 29 mL/min/1.73 m 2 ) and
Stage five: kidney failure (< 15 mL/min/1.73 m 2 ).

Older people are particularly susceptible to kidney damage from age-related decline in glomerular filtration as well as kidney damage from chronic disease states such as diabetes mellitus, hypertension, glomerular, and tubulo­interstitial disorders. The classification of CKD is applicable to all age-groups despite the fact that GFR gradually declines with age. Whether this decline is part of a normal ageing process is uncertain. Age-related decline in GFR is associated with impaired concentrating ability, global glomerular and vascular sclerosis and tubular atrophy with thinning of renal cortex and reduction in kidney size. These changes are considered pathological if observed in younger individuals.
