People with chronic kidney disease (CKD) often have trouble falling asleep and staying asleep. Your physical and mental health can contribute to your sleep problems. Common causes for disrupted sleep patterns in kidney disease patients include: restless leg syndrome, sleep apnea, inadequate dialysis clearance, emotions, changes in sleep patterns, and caffeine.

Restless leg syndrome

Some renal patients wake up in the middle of the night because of an uncomfortable sensation they feel in their legs. Their legs feel itchy or tingly, or may jerk or move on their own, waking them up. People may even experience leg cramps while they are awake, usually during a hemodialysis session. This is called restless leg syndrome. It often occurs when the legs are at rest, such as when a person is sitting or lying in bed.
Mild cases of restless leg syndrome occur periodically, while moderate to severe cases can occur regularly. The sensation varies from patient to patient. It can be irritating, itchy or painful. Some patients find that moving their legs makes the uncomfortable sensations lessen or go away entirely. Because restless leg syndrome occurs when you are trying to relax or fall asleep, it can leave you feeling tired from lack of rest.
Doctors have pinpointed several factors that can contribute to restless leg syndrome. They are:

Emotions

The Prognosis of Focal Segmental Glomerulosclerosis

What does Focal Segmental Glomerulosclerosis (FSGS) mean?

Sclerosis means "scarring to the filters of kidneys, glomeruli". The word "focal" is added because in FSGS, only some of the glomeruli become scarred. "Segmental" means that only some sections of the glomerulus becomes scarred, just parts of them.
So Focal Segmental Glomerulosclerosis means:
Focal = some
Segmental = sections
Glomerulo = of kidney filters (glomeruli)
Sclerosis = are scarred
The prognosis of Focal Segmental Glomerulosclerosis(FSGS) is poor. It is indicated by data that for patients with FSGS, the 5-year survival rate of kidneys is 70%, while the 10-year survival rate is 40%.
For Focal Segmental Glomerulosclerosis(FSGS) patients who can remit completely by treatment, the condition is stable; on the other hand, for FSGS patients who can not remit, the kidney function will worsen constantly, and most of the patients(30%~63%) will develop into Kidney Failure.
The prognosis of FSGS is mainly relates to the severity of the proteinuria, the degree of the pathological changes of the kidney and the response to the treatments. For example:
1. For FSGS patients with persistent Nephrotic Syndrome, the 10-year survival rate is only 45%, while for FSGS patients without Nephrotic Syndrome, the survival rate is over 90%.
2. For FSGS patients with severe hyperlipidimia, if the 24 h urine protein quantitation exceeds 10 g, the kidney function will decrease sharply, and it is called Malignant Focal Glomerulosclerosis. The prognosis of this type is very poor.
3. For patients with FSGS, the prognosis is poor, if the pathological change is accompanied by diffuse mesangial proliferation, obvious arteriolosclerosis and renal tubular interstitial infiltration of inflammatory cells and interstitial fibrosis changes.
4. For patients with FSGS, the prognosis is also poor, if the treatment of hormones has no effects, and the proteinuria and the other clinical symptoms can not remit.

After receiving a kidney transplant, patients with FSGS may show relapse of Nephrotic Syndrome. And pathological changes of focal segmental glomerulosclerosis will occur in the transplant kidney, and the incidence can reach up to 40%. The factors which can lead to the relapse of FSGS are large quantities of proteinuria, the sharp worsening of the condition and the occurrence of diffuse mesangial proliferation. In addition, if women with FSGS who have kidney damages want to conceive, they will easily suffer from pre-eclampsia and the worsening of kidney function.

I would like some help on something I could do to help my feet. The doctor said it was restless leg syndrome.

You obviously have enough to contend with already with your renal failure, and I think you are absolutely right in suggesting that your underlying problems have something to do with the restless leg syndrome in your case and that it could well calm down once your dialysis has started.
My research on your formidable list of medications tells me that tacrolimus can cause pins & needles and sensory disturbances.
Fluvastatin is contraindicated , in other words should not be taken, when there is severe renal impairment and when there is unexplained aching in the muscles, muscle tenderness or weakness – this should be reported to your specialist immediately.
Doxazosin can also cause fluid retention, which is a possible pre-disposing cause of RLS, and amlodipine can also cause muscle cramps.
This however is a calcium antagonist type of drug that strangely enough is sometimes used to treat RLS, so this is unlikely to be the cause.

Although patients with diabetes are regularly monitored for a variety of complications, such as neuropathy, nephropathy, and retinopathy, Hb concentrations frequently are not routinely assessed. Interestingly, reductions in Hb often occur before the onset of overt diabetic nephropathy.
This reduction in Hb occurs for a variety of reasons. Approximately 90% of the hormone erythropoietin is produced by the kidneys. Under normal physiological conditions, hypoxia in the kidney leads to an increase in the production of erythropoietin, which subsequently stimulates erythropoiesis.8 The kidney, in turn, senses increased oxygenation because of the formation of the new erythrocytes and decreases erythropoietin production. However, tubulointerstitial damage associated with diabetes occurs early in the course of diabetes, even before a reduction in GFR or albuminuria is noted.9As functional renal tissue declines in patients with CKD, the body is unable to produce adequate amounts of erythropoietin in response to hypoxia in the kidney.8 Another factor commonly seen in patients with diabetes is the use of medications that may adversely affect Hb production. These include metformin, fibrates, thiazolidinediones, and angiotensin-converting enzyme inhibitors. Finally, systemic inflammation associated with microvascular disease in patients with diabetes leads to the production of inflammatory mediators, such as interleukins and tissue necrosis factor. These mediators blunt the effect of erythropoietin on the bone marrow, where erythroid precursors are stimulated.9
Other factors, although not specific to patients with diabetes, further exacerbate anemia in patients with CKD. These include platelet dysfunction leading to an increased risk of gastrointestinal bleeding, shortened erythrocyte survival time (30–60% of the normal 120 days), and hemolysis secondary to uremic toxin accumulation. In patients receiving dialysis and especially those on hemodialysis, chronic blood loss resulting from frequent phlebotomy for laboratory studies and loss of blood in the dialysis tubing and dialyzer after each hemodialysis treatment may also contribute to declining Hb values. Finally, malnutrition and deficiencies of iron, folate, and vitamin B12have been found to cause a reduction in Hb concentrations.10

Impact of Anemia

The impact of anemia on patients with CKD is profound. In addition to the well known symptoms of fatigue, dizziness, and shortness of breath, anemia has been associated with more severe adverse outcomes, such as cardiovascular complications including left ventricular hypertrophy and congestive heart failure. In patients with diabetes, anemia has been associated with a decline in kidney function, which often occurs in patients with diabetes. Hypoxia caused by anemia stimulates the renin-angiotensin-aldosterone system and contributes to renal vasoconstriction. These factors further exacerbate proteinuria by increasing protein in the renal tubules in patients with diabetes.9 Of note, in patients with type 2 diabetes, anemia has been shown to be an independent risk factor associated with the loss of kidney function.11 In patients with diabetes, anemia can also contribute to the severity of cardiovascular disease and independently increase the risk of retinopathy. Anemia is also thought to hasten the progression of diabetic neuropathy.11 Other general complications associated with anemia include reduced cognitive function and mental acuity, impaired quality of life, and the need for blood transfusions.12–15
Correction of anemia has been shown to improve cardiac function possibly by reducing exercise-induced myocardial ischemia.12Treatment of anemia associated with CKD has also been shown to result in improvements in exercise capacity; physical performance features such as endurance; energy; and physical mobility.13 Patient satisfaction increases when anemia is corrected, as evidenced by higher quality-of-life scores, improved sexual function, better cognition, less depression, and better socialization.14 In non–dialysis-dependent CKD patients, stabilization of renal function has been associated with treatment of the anemia of CKD.12 Finally, treatment of anemia has been shown to reduce hospitalization and mortality rates.

As a result of the potentially severe consequences of anemia in CKD, early recognition and management of anemia are imperative. Consequently, monitoring Hb and detecting anemia in patients with diabetes is essential.
